Major manmade disasters with high death tolls in Africa include explosions, mining, plants, aviation, stampedes, rail collisions, and structural collapses.

Many risk management researchers have pointed out that individuals do learn from their own mistakes, and from risk assessment errors that they experience first hand, while groups, like companies, industrial sectors, states and economies remain focused on foreseeable gain, and accept unreasonable risk levels despite the lessons of history.

A set of Wikipedia lists make alarming reading. Disasters occurring in Africa are listed here, citing first the known death toll, in order of relative magnitude of fatality, then site, incident type, country and date.
